Grace is a Senior Social Prescriber and team manager based in Westminster. Her journey into social prescribing began when she returned to education as a mature student to study Sociology. During her studies, she became particularly moved by the evidence linking loneliness and social isolation to poor health outcomes, which inspired her to pursue a career focused on connection, empowerment, and community support.
Grace works in partnership with both One Westminster, a local charity, and Central and North West London NHS Foundation Trust, delivering social prescribing to individuals supported by Community Mental Health Teams. Her work bridges the voluntary and health sectors, ensuring a holistic and person centred approach to wellbeing.
In addition to her leadership role, Grace has developed a focus on supporting individuals living with chronic pain, as isolation can be more complex in these cases. She created a dedicated resource that is shared with patients, which provides guidance across four key areas: psychological support, community connection, symptom tracking, and understanding pain.
Grace has lived with chronic pain for much of her life, having been diagnosed at a young age with conditions such as Endometriosis, Osteitis, and Hypermobility Syndrome. Her firsthand experience informs her compassionate approach and drives her passion for helping others find ways to live with pain. She particularly enjoys supporting people to rediscover joy, purpose, and personal interests—believing these can be powerful tools in pain management and overall well being.
